max_file_size not enforced?

I upgraded from version 2.x to 3.x and did not change any of the filter setup for `max_file_size`.  I am finding that with the same setup for version 3.x, the max_file_size error is no longer raised if the file is too large. Was there a change in the setup or was a bug introduced in 3.x?
